New Employee Forms
In the table below you will find all of the New Hire Documents that need to be completed to start your employment. To complete the I-9 Verification Form, Federal guidelines require that photo identification is presented in person. The name you provide on all employment forms must be your name EXACTLY as it appears on your Social Security Card. All completed forms and photo identification should be presented to Human Resources (3rd Floor, Thompson Hall). An employee handbook will be provided to you during your first week of employment.
